REQUIREMENTS TO EXPEDITE A CHILD PASSPORT

INSTRUCTIONS:

  
• Complete  DS-11passport application form, fill it online. But do not sign.
• Two passport photos. 2" x 2" white background. (Photo service is available at our office.)
• Original or certified copy of birth certificate.
• Children under 16 years old - both parents and child need to present at post office or county clerk when executing your child's passport application. If only one parent can be present, the un-accompanied parent needs to complete a DS-3053 application and have it notarized.
