Visa Requirements for U.S. Citizens

 TOURIST

• Valid and signed U. S. Passport (valid at least 6 months beyond the date of entry; with at least one blank visa page)  – photocopy is not acceptable

• One Visa Application Form completed and signed. Application on thermal fax paper is not accepted. The application form must have original signature.

• One Passport Type Photograph

• World Health Certificate required showing valid vaccinations against Yellow Fever 

• Computer generated flight itinerary from the travel agent or copy of roundtrip airline ticket

• Service Order Form

 BUSINESS

OFFICIAL or DIPLOMATIC PASSPORT HOLDERS ONLY

• U.S. Official or Diplomatic Passport, at least 6 months remaining validity
• Three Visa Application Forms completed and signed. Applications on thermal fax paper are not accepted. The application forms must have original signature. 
• 3 Passport-type photographs required
• Letter from sending Government Department or Agency, addressed to " Embassy of Benin, Visa Section, Washington D. C. " detailing the applicant's name and occupation, Purpose of the trip, Dates of the trip and contact person in Benin
• World Health Certificate required showing valid vaccinations against Yellow Fever 
• Service Order Form

Consular Fee:       $100.00/visa (effective January 1, 2004)
