Visa Requirements for U.S. Citizens

 TOURIST

• Valid and signed U. S. Passport (valid at least 6 months beyond the date of entry; with at least one blank visa page)  – photocopy is not acceptable

• Two Visa Application Forms and Work History Form completed and signed. Application on thermal fax paper is not accepted. The application form must have original signature.

• Three passport type photographs

• Report Arrival Form

• Computer generated flight itinerary from the travel agent or copy of roundtrip airline ticket

• Service Order Form

 BUSINESS

• Valid and signed U. S. Passport (valid at least 6 months beyond the date of entry; with at least one blank visa page)  – photocopy is not acceptable

• Three passport type photographs

• Report Arrival Form

• Invitation letter from host in Myanmar (if available)

• Computer generated flight itinerary from the travel agent or copy of roundtrip airline ticket

• Business Letter from U.S. Company

• Service Order Form

OFFICIAL / DIPLOMATIC

• Submit your U.S. Official or Diplomatic Passport, must have at least 6 months remaining validity
• 3 Visa Application Forms, fully completed and signed
• 4 Passport-type photographs 
• Service Order Form
• Letter from sending department or agency, addressed to:
  
  Embassy of Myanmar, Visa Section, Washington D.C.
  
  The letter must include all of the the following information:
• Applicant's position and title
• Purpose of travel
• Dates of the trip
• Contact person in Myanmar

Consular Fee:       $20.00/visa (Tourist)

                                    $30.00/Visa (Business)

                                    Gratis (Official or Diplomatic Passports ONLY)
