For diplomatic or official travel, visa is required.  Please submit the following:

  • Submit your valid U.S. Official or Diplomatic passport with at least six months remaining validity
  • One Visa Application Form, completed and signed
  • One Passport-Type Photograph 
  • Service Order Form
  • Visa Cover Letter from the U.S. Department of State
