Philippines – Passport is
Required. Visa is NOT required for tourist or
business purposes if the duration of stay is 21 days or
less. Visa is required for over 21 days stay.
Important
NOTE: The Embassy of the
Philippines is jurisdictional and will process visas only for
residents of Alabama, D.C., Florida, Georgia, Kentucky,
Maryland, North Carolina, South Carolina, Tennessee, Virginia
and West Virginia. Residents of other states should contact
at 1-866-886-8472 for the visa
application form within that jurisdiction.

TOURIST

Valid and signed U. S. Passport
(valid at least 6 months beyond the date of entry; with at least
one blank visa page) – photocopy is not acceptable

One Visa Application Form
completed and signed. Application on thermal fax paper is not
accepted. The application form must have original
signature.
The application form
must have original signature and must be notarized.

One Passport Type Photograph (Important:
Please write your name and sign on the front bottom of the
photograph)

Computer generated flight itinerary from
the travel agent or copy of roundtrip airline ticket

Notarized
parental consent from the non-traveling parent(s) - applicable
for applivants under 18 years old.

Proof
of Financial Support from the applicant (current bank
statement).
NOTE:
For minor applicants (under 18 years old), the parents must
write a notarized affidavit of support for the minor and
also copy of their current bank statement.
