one blank visa page) – photocopy is not acceptable

completed and signed. Application on thermal fax paper is not
accepted. The application form must have original
signature.

Business letter written on your
company’s letterhead addressed to the Embassy of Tanzania. This
letter should indicate the applicant’s name, position in the
company, nature of business to be conducted in Tanzania, name
and address of contact in Tanzania, length of stay and the
number of entries requested. It should also state that the
company would be financially responsible for the applicant’s
living expenses including roundtrip transportation, and should
be signed by someone other than the applicant.
