• Valid and signed U. S. Passport (valid at least 6 months beyond the date of entry; with at least one blank visa page)  – photocopy is not acceptable

• One Visa Application Form completely filled out and signed (original signature required)

• One Passport Type Photograph (with clear or white background)

• Service Order Form

 BUSINESS

• Valid and signed U. S. Passport (valid at least 6 months beyond the date of entry; with at least one blank visa page)  – photocopy is not acceptable

• One Visa Application Form completely filled out and signed (original signature required)

• Business Letter from U.S. company

• One Passport Type Photograph (with clear or white background)

• Service Order Form

Consular Fee:        $58.00/visa (Single Entry)

                                     $194.00/visa (Multiple Entry - 1 year multiple entries)
