Valid and signed U. S. Passport
(valid at least 6 months beyond the date of entry; with at least
one blank visa page) – photocopy is not acceptable

completed and signed. Application on thermal fax paper is not
accepted. The application form must have original
signature.

International Certificate of Vaccination for Yellow Fever

Computer generated flight itinerary from
the travel agent or copy of roundtrip airline ticket

Business letter written on your
company’s letterhead addressed to the Embassy of Uganda. This
letter should indicate the applicant’s name, position in the
company, nature of business to be conducted in Uganda, name and
address of contact in Uganda, length of stay and the number of
entries requested. It should also state that the company
would be financially responsible for the applicant’s living
expenses including roundtrip transportation, and should be
signed by someone other than the applicant.
