Requirements to Expedite A Lost 0r Stolen Us Passport

Instructions:

  

• Complete  DS-11 passport application form, fill it online. But do not sign.

• Original/certified US birth certificate or Original naturalization certificate.

  

• Proof of Identity (A Valid Driving License, Government Issue Photo I.D).

  

• If your passport was lost or stolen: complete form DS-64, must be signed and dated.
