Name Change U.S. Passport
REQUIREMENTS TO EXPEDITE U.S. PASSPORT NAME CHANGES
INSTRUCTIONS:
- If the change is within one year after issuance, please fill out form online DS-5504 completed and signed. If the temporary passport was issued more than one year, please fill out form DS-82 online.
- Two passport photos. 2" x 2" white background. (Photo service is available at our office.)
- Proof of Identity (A Valid Driving License, Government Issue Photo I.D).
- Original or certified true copy of the document showing the name change (example: Marriage License, Court Order, Divorce Decree, etc.)

- Complete AA Passport and visas service order form
