Requirements to Expedite U.S. Passport Name Changes

Instructions:

  

• If the change is within one year after issuance, please fill out form online DS-5504 completed and signed. If the temporary passport was issued more than one year, please fill out form DS-82 online. 

  

• Two passport photos. 2" x 2" white background. (Photo service is available at our office.)

  

• Proof of Identity (A Valid Driving License, Government Issue Photo I.D).

  

• Original or certified true copy of the document showing the name change (example: Marriage License, Court Order, Divorce Decree, etc.)

• Complete AA Passport and visas service order form

Before you send us your application with the sealed package through FedEx / UPS/DHL or any other mail service provider, you must contact AA Passport and Visa service at (469) 835-2731 to confirm your rush or emergency passport space. Our services are guaranteed.
